Step 2– Once you click on the link provided above you will be directed to a new webpage that displays the application form below. Complete the personal information section with the following details.

• H-D Employee Name
• H-D Dealer Number
• First Name
• Last Name
• Suffix
• Date of Birth
• Social Security Number
• Country of Citizenship
• Select the “Yes” option if you are a citizen of another country
• Country of Permanent Residence
• Primary Phone
• Mobile Phone Number
• Email Address

Step 3– The Housing Information section requires the following details.

• Street Address
• APT/ Unit
• City
• State
• Zip Code
• Time at Address
• Housing Status
• Monthly Payment
• Alternate Mailing Address

Step 4– Proceed to the Income and Employment section of the form. Provide the following information in the blank spaces below.

• Employment Status
• Employer
• Occupation
• Annual Income
• Other Income

Step 5– Select whether you have a checking or savings account, check the “Yes” option if you are interested in balance transfers and then check the “Yes” option to add an authorized user.

Step 6– View and accept the Harley-Davidson Visa Card Terms and Conditions by checking the box below. Finally, click on the “Submit” and you will receive a success message. You will be informed about your application status once the Bank review and verify the information provided by you.
