How to Apply
Step 1– In order to apply for the CareCredit Card, you must open the application homepage (carecredit.com). To get started select a doctor or business by name, select a doctor or business by location or select a market where you hope to use the card.
Step 2– Press “Next” and you will be taken further to a new web page that displays the application form. View the terms and conditions, before applying.
Step 3– Complete the form with the following details:

- Last Name

- Apt#
- Zip Code
- Primary Phone Number
- Additional Phone Number (select type)
- Email Address (confirm)
- Social Security Number
- Date of Birth

Step 4– Select the “Yes” option if you wish to add a joint applicant and then choose a statement delivery method.
Step 5– Read and agree to the optional card security debt cancellation agreement and then press “Continue”.
