How to Apply

Step 1– To apply for the Blaze Credit Card visit the application homepage (blazecredit.com). Scroll down and you will find the application form. Complete the personal information section with the following information:

  • First Name
  • Last Name
  • Address
  • City
  • Date of Birth
  • Social Security Number
  • Mother’s Maiden Name

Step 3– The Income & Financial Information section requires the following information:

  • Taxable Income
  • Nontaxable Income
  • Select whether rent or owns
  • Monthly Rent or Mortgage obligation

Step 4– Finally, click on the “Next” button to submit the application form. Once your application is approved you will be sent a confirmation message.

Check Application Status

Step 1– If you have already applied for the credit card you can check your application status by calling at 1-605-782-3472.

Cancel Credit Card
